“The Palm Beach County TRIAD OF VIAL OF LIFE”
The Palm Beach County Sheriff’s Office has requested the C.O.P volunteer units to distribute “The Palm Beach County TRIAD OF VIAL OF LIFE”. Its purpose is to establish a medical record of the conditions and medications as it relates to a senior citizen. This record can be a life saving tool for paramedics when they arrive at a medical call. Included in the kit is a Green Decal that is posted on the entry way to your home. This Decal alerts the paramedics that you have the Vial of Life with the medical information posted on your refrigerator door. Each person in your household should fill out the Vial of Life form. POST THE GREEN DECAL IN YOUR ENTRY AREA OF YOUR HOME. Vials of Life forms are available at the home of Bernice & Bernie Maltz.

Thank you. Bernie Maltz
